Media Company Service Agreement Insertion Order (“IO”) Form

This Insertion Order for the Media Company Service Agreement is designed to be used solely for the retention of a Publisher to provide commercial email marketing. The Insertion Order, often referred to as an “IO”, sets forth specific information regarding the campaign including pricing, budget, the authority to use affiliates or sub-publishers, the geographical limitations (if applicable), and the contact information for the Publisher, referred to as “Media Company” and the Advertiser, referred to as “Customer”.
